You and your partner (if applicable) will need to fill out an intake questionnaire on our patient portal prior to your first appointment.

This must be completed prior to your appointment, otherwise your appointment will need to be rescheduled.

At your first visit, one of our physicians will outline a plan for fertility investigations and/or treatments. All of our treatments are outlined under “Fertility Services”.
